San José may be kind of dumpy as a city, but it contains some world-class medical facilities, and several of these, most notably the Clínica Bíblica, have made it a specialty to cater to foreigners, who can get most procedures at a fraction of what it costs them at home; there's also a number of small hotels and guest houses which specialize (sometimes exclusively) in post-op stays. Another reason Costa Rica has become popular for American citizens is its proximity to the U.S.
